At Allergy & Asthma Associates of Michigan we know managing all your care needs, from routine health maintenance to advice from specialists and changes in medications, can be complicated. It does not have to be.
Chronic Care Management, also called CCM, is a program available to Medicare patients living with more than one chronic condition, such as arthritis, diabetes, depression, asthma, high blood pressure or other long-term conditions.
Care Coordination
Allergy & Asthma Associates of Michigan offers CareHarmony care coordination to help patients stay supported between regular office visits. Patients benefit from monthly telephone assistance and a dedicated Care Coordinator who helps guide them toward better health.
This version emphasizes asthma, allergy and clinical immunology care, including patients who need ongoing support with medications, biologics, avoidance strategies and care coordination across providers.
How this can help patients at Allergy & Asthma Associates of Michigan
- Moderate to severe asthma: Monitor symptoms, reinforce inhaler and biologic adherence, identify worsening control early, and help reduce exacerbations and hospitalizations.
- Chronic allergy patients: Support patients with allergic rhinitis, immunotherapy, food allergies and drug allergies by improving treatment adherence, reinforcing avoidance strategies, and coordinating care across the continuum.
- Multiple chronic conditions: Help patients managing asthma along with COPD, diabetes, obesity or GERD stay engaged with treatment plans and connected to the right providers.
What your Care Coordinator can help with
Chronic Care Management extends care outside of the four walls of your physician’s practice to help you maintain the best possible health. Each month your Care Coordinator can call you to:
- Assist with scheduling appointments, lab tests or other tests.
- Explain how and when to take your medications.
- Coordinate any home health or medical equipment needs.
- Connect you with health education resources, services and programs.
- Identify available community resources.
- Coordinate follow-up care after leaving the hospital.
- Access other support services you need to stay well.
There is also a 24/7 Care Coordinator Phone Line to help answer questions day or night outside of your monthly calls with your Care Coordinator.
Cost and Coverage
This service is covered by Medicare and is subject to your annual deductible and coinsurance, usually 20%. If you have a secondary or supplemental insurance plan, your coinsurance may be covered. It is best to review exactly what your insurance plan covers.
You can only sign up for Chronic Care Management services with one CareHarmony care coordination team at a time. You may opt out any time. There are no strings attached.

Chronic Care Management is a care coordination program designed by Medicare to help patients better manage chronic conditions. It gives you greater support and access to care between office visits, even while at home.
A chronic condition is an ongoing, long-lasting health condition that requires continual management and treatment. Examples include asthma, diabetes, arthritis, hypertension and heart disease.
Care coordination is the organization of patient care activities and the sharing of information among the people involved in a patient’s care. CCM provides extra layers of support between doctor visits for eligible patients with two or more chronic conditions.
To be eligible for Chronic Care Management, you must be a Medicare beneficiary and have two or more chronic conditions expected to last at least 12 months, or until end of life. If you are unsure whether your conditions qualify, contact your provider for guidance.
Yes. At this point the program is only being offered to Medicare patients.
Managing and coordinating care can be difficult if you have multiple chronic conditions, see different types of doctors, or take several medications. Coordinated care helps your doctors get the information they need, when they need it.
One benefit of Chronic Care Management is helping you maintain good health. The program can help you keep up with preventive care, connect with specialists, locate healthcare resources, and access community services.
Many Medicare patients have secondary insurance, which often covers CCM copays. If not, CCM may be subject to coinsurance, usually 20%, as well as your deductible. It is best to review exactly what your insurance plan covers.
Yes. The same rules that protect your medical information in your doctor’s practice apply here as well. Your information is secure and handled according to HIPAA guidelines.
Chronic Care Management extends your care beyond office visits. You will be matched with a dedicated Care Coordinator and have access to support 24/7. Your Care Coordinator can help with scheduling, refills, lingering questions, treatment planning and communication across your care team.
